Sharing your own or your loved one’s JdVS diagnosis and the unique characteristics associated with JdVS can be overwhelming. Sometimes it can be hard to know where to start and what to share. To assist families in sharing their story and providing professionals with the necessary background information about their child, we have created this template. While this template was created for a school aged JdVS individual, it can be easily edited to be useful for all ages.
